Sam levy was born in Liberty city to Ivan Levy (Formerly Bagaslavsky) and Susan Levy on September 1st 1995. Sam's father immigrated to from Russia, and his family had to change the family name to a more "American" name. When asked what to change it, Sam's Grandfather saw someone wearing a pair of Levi's and thought they had meant Levy, hence the spelling and pronunciation. The levy's had lived in poverty, taking loans from many loansharks to cover needs. when they failed to pay, Sam was kidnapped and was attacked by the sharks as an example, and is still left with three scars on his chest.

Sam is the Attending Gastroenterologist at Mount Zonah Medical Center, and is known to be a workaholic. He is known to be very compassionate towards his patients, and is mainly focused on Chronic cases, partially to find a solution that works for his chronic gi conditions himself.
